The password of Form 16 Format as follows

The password will be your PAN (first 5 letters in uppercase) and date of birth  (“DDMMYYYY” format). For example, if your PAN is ABCDE1234M and the date of birth is 01/Dec/1998, then the password will be “ABCDE01121998”.
